Services we offer

PRIMARY HOME CARE (PHC)

PHC is non-technical medically related personal care service that is available to eligible adults and high-risk pregnant women. Persons with health problems that limit their functions in performing activities of daily living qualify accordingly to a practitioner's statement of medical need. Services provided are by a primary home care attendant.

FAMILY CARE (FC)

Family Care is a non-skilled, nontechnical attendant care service available to eligible adults who are functionally limited in performing by an attendant and do not require supervision of a registered nurse.

PERSONAL CARE SERVICES (PCS)

ESCORT

Accompany patients to medical appointments for either medical treatment or diagnosis.

HOME MANAGEMENT

Assistance with housekeeping activities that support the patient's safety and health.

Laundry
Shopping

PERSONAL ASSISTANCE

Assistance with activities such as:

Bathing
Dressing
Meal Preparation
Exercise
Feeding/Eating
Grooming/Shaving/Oral Care
Routine Hair and Skin Care
Toileting
Assistance with Administering Medications
Transfer and Ambulation
Walking

Eligibility Requirements

Those who have a disability or medical needs, adults and elderly over the age of 65 with a disability or medical needs due to health issue qualify for services. Patients with Medicare, Medicaid, and all private insurances located in the Rio Grande Valley, including the counties of Cameron, Willacy, Hidalgo, Starr, Zapata, Webb, Bexar, Brooks and Nueces qualify for services.
